Died:  10 September 1916, Salt Lake City, Salt Lake, Utah (Pulmonary Tuberculosis)

Buried:  13 September, 1916, Salt Lake City Cemetery, Salt Lake, Utah; location:  Plot O, Block 14, Lot 6.
